Submission Requirements:
Completed application form with required signatures. Legal owner(s) must sign the application.

A scaled drawing accurately describing the location of all existing and proposed structures in relation to the property lines, the location of the well and septic (if
applicable), the lines and distances of each property line, the location of any easements or rights-of-way affecting the property, and the location and width of the frontage street/road. (Certified Survey)

 Copies of any deed restrictions affecting the property.

A statement of the Nature of Request and an explanation of the Exceptional Practical Difficulty  - why it is not possible for the proposal to meet the Code requirements. Attach a separate letter.

The Division will provide notice of the pending application to the adjacent property owners and the site will be posted in advance of the meeting date.
